If you want to send your book to be reviewed by Story Doctor, here is how to do it.

Genre: [Mention the main genre. You can add a few more sub-genre or theme-tags.]
Age Category: Adult/ YA
Word count:
Format: [I don't mind eBooks at all; I can read directly from Kindle, if it's available in KU]
Publisher Name: [If it's self-published, you write 'Self-Published' here]
Deadline: [Let me know when it'll be published or if you have a deadline.]

Number of Critiques your manuscript passed through: [Doesn't have to be professional or paid. It can be critiqued by your critique group members or other critique websites where you can get many critiques for free. Getting a manuscript critiqued is essential. Here, write the rough number of critiques your book passed through while at the Manuscript level if you're self-published]
Book Cover: [Add it in the Body of the email NOT as an attachment; to do that simply drag the image from your pc and drop it to the body of the email, resize and format it to a good visual if needed. No attachment will be opened in the first email]
Blurb/Pitch: [Here, don't give shady/ ambiguous lines like ... but it was only the beginning ... she didn't know what's coming ... but there are more ... and there's a world to save ... Here, you simply "hook". Shady lines don't hook. Start from after there-are-more. This is in between short pitch and short summary [200 words].
Bio: [Now, I really don't care about in which town or city you live, if you have a family or kids or not. I don't care if you go to the local library every day or not. You wrote a book, and I'm already presuming that you love books. We all love books. We are in the same family since we share the book-lovers'-gene. No point mentioning where you graduated or what degrees you have. Your first 10 pages and blurb will tell me how much studies you have done around creative writing and publishing. What I DO WANT TO KNOW are: Are you a multi-talented person? Making short films while writing at the same time? Doing archery or other sports, too? Traveled museums in Cairo and wrote fiction about it? Did Kung Fu with monks? Wrote the book while you were/are in prison? Survived a marriage when you were a child in a village in India? A girl who lived in the mountains of Tibet? Wrote the book while fighting chronic disease and you don't have much time on the earth?
Mesmerize me with your life story in a line or two.
